Photography / Model Release Form
I hereby consent to be photographed, filmed, videotaped by an employee or
representative of Wild Attire, Inc. I understand any such photograph, film, videotape or
audiotape may be used in exhibits, advertising publications, web site, marketing materials,
educational materials, publicity, promotion, editorial or illustration or for any other purpose
that Wild Attire, Inc. deems appropriate.
I authorize Wild Attire, Inc. to use, reuse, publish or republish in any medium, for any purpose, at any time, this photograph, film videotape or audiotape, and to use my name in conjunction with them at its sole discretion.
I acknowledge that by signing this form I give up all claim of ownership of the photographs. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use. I agree that in the event this form is determined by a court, not to be a work for hire agreement under the federal copyright laws, this form shall operate as an irrevocable assignment by myself to Wild Attire, Inc. of the copy, trade and service mark rights to the Videos, including all rights there under, in perpetuity. Under this irrevocable assignment, I hereby assign to Wild Attire, Inc. the sole and exclusive right, title, and interest in and to the videos, without further consideration, and agrees to assist Wild Attire, Inc. in registering and from time to time enforcing, at Wild Attire, Inc.'s expense, all copyrights and other rights and protections relating to the videos in any and all countries.
I understand that this consent cannot be revocated, and I release Wild Attire, Inc., its employees, and/or agents from any claims or lawsuits, arising out of the use of my photograph, films, videotapes or audiotapes or the use of my name.
I hereby release, remise, quitclaim, and forever discharge the Wild Attire, Inc., its officers, directors, shareholders, employees, agents, partners, attorneys, affiliates, successors and assigns from all actions, claims for relief, causes of action, suits, debts, dues, sums of money, bonds, bills, specialties, covenants, contracts, controversies, agreements, promises, variances, trespass, damages, judgments, extents, executions, claims, liens, and demands whatsoever, in law, admiralty, or equity which I ever had, now has, or hereafter can, shall, or may have against Wild Attire, Inc., its officers, directors, shareholders, employees, agents, partners, attorneys, affiliates, successors and assigns, by reason of the this form. This form is absolute and unconditional and contains the entire agreement between Wild Attire, Inc. and me. This form may not be changed or otherwise modified in any way without a writing signed by both Wild Attire, Inc. and myself. All understandings and agreements between Wild Attire, Inc. and myself are merged into this form. Wild Attire, Inc. and myself are not relying on any statement, representation, warranty, or other promises, express or implied, oral or written, in fact or in law, made by Wild Attire, Inc. or other person, except as stated herein.
